Preparing For Your Office Visit
There
are a few important things you can do to help your office visit run
smoothly. In order to provide you with the best possible care, it is
important for us to know your medical history. You should complete a
medical history form (This will be mailed to you when you schedule your
first appointment) and bring it with you to your first appointment. In
addition, you should obtain a referral from your Primary Care Doctor if
required by your insurance.
PLEASE BRING YOUR MOST RECENT OR
FAVORITE GLASSES WITH YOU EVEN IF THEY ARE BROKEN OR UNSATISFACTORY.
This helps tremendously in our ability to make the best recommendations
for new glasses, if needed.
